A chronic medical condition is one that has been (or is likely to be) present for six months or longer (for example, asthma, cancer, heart disease, diabetes, arthritis, stroke, depression).
Would you benefit from seeing health professionals ( ie specialist or allied health provider – e.g, physiotherapist, podiatrist, osteopath) other than your GP?
If you would need to access the services of 2 or more health providers other than your GP, your doctor may be able to create a GP Management Plan (GPMP).
This plan then entitles you to 5 medicare rebateable visits with an allied health practitioner per calender year.
This plan can help people with chronic medical conditions by providing an organised approach to care. A GPMP
• Identifies your health and care needs;
• Sets out the services to be provided by your GP; and
• Lists the actions you can take to help manage your condition.
